What Is Xiaflex®?

Xiaflex is the first and only FDA-approved non-surgical treatment for Peyronie’s disease in adult men with a palpable plaque and curvature of at least 30 degrees. The medication contains collagenase clostridium histolyticum—a specialized enzyme that works by breaking down the excess collagen that forms the Peyronie’s plaque. When injected directly into the plaque, Xiaflex helps dissolve the fibrous scar tissue responsible for penile curvature, allowing the body to gradually absorb and eliminate it over time.

How Xiaflex Treatment Works

Xiaflex therapy follows a structured treatment protocol administered over multiple cycles:

  • Injection Sessions: each treatment cycle consists of two injections given one to three days apart, with the medication delivered directly into the Peyronie’s plaque
  • Penile Modeling: one to three days after the second injection, Dr. Nourparvar performs an in-office modeling procedure—a manual stretching technique designed to help elongate the treated plaque and improve curvature correction
  • Home Exercises: patients are instructed to perform daily penile stretching and straightening exercises at home for six weeks following each treatment cycle
  • Multiple Cycles: up to four treatment cycles (eight total injections) may be administered, with at least six weeks between cycles to allow for tissue response and healing

Who Is a Candidate for Xiaflex?

Xiaflex is indicated for adult men with stable Peyronie’s disease who meet specific clinical criteria. Ideal candidates typically present with:

  • Palpable Plaque: a distinct area of scar tissue that can be felt during physical examination
  • Curvature of 30 Degrees or Greater: penile curvature that is significant enough to affect sexual function or cause distress
  • Stable Disease: curvature that has not changed significantly over the past three months, indicating the condition has moved beyond the acute inflammatory phase

What to Expect During Treatment

Xiaflex injections are performed in our private Los Angeles office within the Cedars-Sinai Medical Office Towers. The injection itself takes only a few minutes and is administered to the flaccid penis. Following the procedure, patients can return home the same day. Some swelling, bruising, or discomfort at the injection site is common and typically resolves within one to two weeks. Patients are advised to avoid sexual activity for at least two weeks after each injection and until any pain or swelling has subsided.
